Join the Hemby Cup Tournament

Thank you for your interest in joining the Hemby Cup tournament. The fee per team is $900. A portion of the registration fee will benefit the Presbyterian Hemby Children's Hospital. To register and submit your team roster:

1. Save the PDF form on the right to your computer. Rename the file as your team name.
2. Please fill in the interactive form fields in Acrobat Reader and save. Since handwriting can be difficult to read, we are requiring that all forms be filled out electronically. We will use the information as submitted.
3. Email the final form to Extreme Ice Center. If you will be paying the fee with a check, please indicate that in your email. If you are submitting payment with a credit card, then your registration is complete!
4. If paying the fees with a check, send a copy of your forms and the check to:
